	It is strongly recommended that the passport for an animal registered with a breed society is obtained from the relevant breed society, not the BDS. In the event that an owner requires a BDS passport for an animal registered with a breed society, the breeding details will have to be completed and stamped by the breed society AFTER THE PASSPORT HAS BEEN ISSUED. Legislation requires breeding details to be stamped by the breed society as the competent authority.

Description	The diagram and description on the back of this form should be completed and signed by a Veterinary Surgeon or you may use a copy of your vaccination certificate or other veterinary identification. Only if your equine is micro-chipped may you complete the diagram/description yourself

It is compulsory to complete this section.
