Completing the passport identification diagram
Please complete the form in block capitals using black
ink, except for white markings on the horse diagram which
should be drawn and hatched in red ink. All white areas on
coloured horses must also be drawn and hatched in red ink.
Make sure that as many identifying marks as possible
are shown. Mark any whorls (spiral patterns of hair) with
an X and scars with a V. This is
particularly important if your horse is predominantly one
colour.
Make sure that a full description of all markings and
features is in the relevant boxes on the form. Please
state if the eyes are not the normal brown colour (e.g.
Wall eye, blue, pink). Also state colour of hooves –
black, white or striped.
Make note of any brands & freezemarks in the box
provided and microchip numbers in the signalment key box.
Note any permanent congenital defects.
