Horse Passport Application Forms

You can print a BDS passport application form from this web site. The form consists of 2 pages and is available in two formats:

Adobe Acrobat format (pdf file) containing both pages.
(requires Adobe Acrobat reader to be installed on your machine)

We recommend that you download the Acrobat (pdf) version of the form. This will produce a printout that most accurately reproduces the original form.

Change of ownership notification
If you sell your horse / pony, use this form to inform the BDS of change of ownership.

Forms not printing properly ? Your settings may not be correct. Click on the File menu at the top of the page, then on Page Setup. Make sure that you have selected A4 as the paper size, portrait orientation and that all margins are set to 10 mm or less. If you are still having problems printing the application form, please contact the BDS office, who will be able to send you a form by post.

Guidelines for filling in passport applications

IMPORTANT - PLEASE NOTE:
The diagram and description on the back of the form MUST be completed and signed by a Veterinary Surgeon. You may send a copy of your vaccination certificate or other veterinary identification. Only if your equine is micro-chipped may you complete the diagram/description yourself

You may send a photocopy of your veterinary record of vaccination showing the diagrams of your equine, together with the description as completed by your vet. You do not need to send the original.

Make sure that you sign section 3 on the form i.e. “not intended for human consumption” or “is intended for human consumption”

Completing the passport identification diagram

Please complete the form in block capitals using black ink, except for white markings on the horse diagram which should be drawn and hatched in red ink. All white areas on coloured horses must also be drawn and hatched in red ink.

Make sure that as many identifying marks as possible are shown. Mark any whorls (spiral patterns of hair) with an X and scars with a V. This is particularly important if your horse is predominantly one colour.

Make sure that a full description of all markings and features is in the relevant boxes on the form. Please state if the eyes are not the normal brown colour (e.g. Wall eye, blue, pink). Also state colour of hooves – black, white or striped.

Make note of any brands & freezemarks in the box provided and microchip numbers in the signalment key box. Note any permanent congenital defects.

 

Blaze: A broad white marking down the centre of face.
Stripe: A narrow white marking down centre of face.
Star: A white mark on the forehead.
Feint: An indistinct star where white hairs are mixed with darker hairs.
  White Limbs: Mark white on limbs in red hatching, you must indicate where the white markings end in relation to the joints e.g white to knee.
 
Whorls: These are spiral patterns of hair that occur where the horses coat changes direction. They are usually small circles but may also be extended. They are most commonly found on the head, neck, chest and flanks. Please mark with an X

 

  Scars: These should be indicated with a letter “V” and an arrow pointing to the location of the scar.
